Combined School & Sports Physical
Overview
An in-person appointment for one child who needs both a school health examination and a sports participation evaluation. Bring both forms and relevant health records. The provider reviews the requirements, performs the appropriate examination, and completes documentation supported by the findings. Additional testing or referrals may be needed; clearance is not guaranteed.
What the exam covers
- Height, weight, and body mass index
- Review of vision and hearing screening needs
- A head-to-toe physical examination
- Review of immunizations and health history
When a mobile visit may be appropriate
- Your school requires an Illinois Certificate of Child Health Examination
- Your child is entering kindergarten or first grade, sixth grade, or ninth grade
- Your child is new to an Illinois school
- You would prefer to avoid a clinic waiting room
How the mobile visit works
- Request a mobile visit and share your address so we can confirm one-way distance from ZIP 60407.
- We confirm the travel fee — one fee per household per visit — along with billing and coverage details.
- Your provider performs the physical examination and reviews immunizations and any screening or testing still needed. Separate screening or testing may require referral.
- You receive the completed exam documentation to submit to your school.
What your provider may evaluate
- Your symptoms, how long you have had them, and what you have tried
- Your personal and family medical history
- Current medications, supplements, and allergies
- Factors that may be contributing to your concern
- Your child's health history and current medications
- The immunization record and any prior exam forms
- Any symptoms or concerns you would like addressed during the exam
Possible treatment options
Treatment options may include prescription or nonprescription therapies when clinically appropriate. Your provider will determine whether a treatment is safe and appropriate after reviewing your health history and completing a clinical evaluation. Prescriptions are never guaranteed.
The visit documents the findings of the examination. Any needed follow-up, additional testing, or immunizations is discussed with you and referred appropriately.
When additional testing or follow-up may be needed
Some concerns may require laboratory testing, imaging, an in-person examination, or referral to a specialist. Your provider will advise you if your situation is not appropriate for virtual care.
Some findings, or immunizations not given during the visit, may require follow-up with another provider or your local health department.
Travel fee and household visits
Travel fee
A travel fee applies only when a visit must be completed in person, such as a school health physical or sports physical that requires a hands-on exam. Virtual visits do not include a travel fee.
| Distance (one-way from 60407) | Self-pay travel fee | Self-pay | Insured service area* |
|---|---|---|---|
| Up to 30 miles (one-way driving) | $75 | ||
| Over 30–60 miles (one-way driving) | $125 |
*Service-area eligibility does not confirm insurance coverage. The dollar amounts above apply to self-pay travel only.
- One travel fee per household per visit — not one per patient. A household with two children pays a single travel fee.
- Mobile visits are available at Illinois addresses only. Self-pay patients may choose either distance tier, measured by one-way driving mileage from ZIP 60407.
- Insured patients may select the up-to-30-mile area only.
- For insured visits within 30 miles, we seek travel reimbursement when the applicable billing rules allow. If the plan does not pay the travel charge, the clinic absorbs it; you will not be billed for unpaid travel. Normal patient responsibility for the clinical visit may still apply. Mobile availability is confirmed before scheduling and may change by plan after reimbursement review.

Frequently asked questions
Can I get a prescription through a virtual visit?
A prescription may be considered only when clinically appropriate and after your provider reviews your health history and completes an evaluation. A prescription is never guaranteed.
Can I receive care if I live outside Illinois?
Not at this time. First Priority Clinic currently provides telehealth services to eligible patients located in Illinois.
Is the travel fee included in the price of the visit?
For self-pay visits, travel is separate: $75 up to 30 miles and $125 over 30–60 miles, measured by one-way driving distance from 60407. One travel fee applies per household appointment. Insured mobile visits are limited to 30 miles; the clinic absorbs unpaid travel charges. Illinois addresses only.
Will my insurance cover the travel fee?
Travel reimbursement is requested where applicable billing rules allow. If the plan does not pay the travel charge, the clinic absorbs it; the patient is not billed for unpaid travel. Usual clinical-visit cost sharing may apply. Mobile availability is confirmed before scheduling.
Will the school accept the completed form?
Acceptance is decided by the school. We complete the exam and the documentation; requirements are set by the school, the Illinois Department of Public Health, and the Illinois High School Association.
How long is the school health exam valid?
Illinois requires the health examination within one year before entry for the required grade levels. Your school may apply additional rules.
