Sports Physical

Overview

An in-person pre-participation sports physical, including the cardiovascular exam some athletic programs require. An IHSA pre-participation examination is valid for 395 days from the date of the exam. Because the exam is hands-on, it cannot be completed by video. We travel to your home by appointment.

What the exam covers

  • Cardiovascular examination, including heart and blood pressure check
  • Height, weight, and review of vision screening needs
  • Review of medical history, medications, and prior injuries
  • Completion of the pre-participation form you provide

When a mobile visit may be appropriate

  • Your school or athletic program requires a physical before participation
  • Your current physical is near or past its 395-day validity
  • Your program requires a cardiovascular exam
  • You would prefer to be seen at home

How the mobile visit works

  • Request a mobile visit and share your address so we can confirm one-way distance from ZIP 60407.
  • We confirm the travel fee — one fee per household per visit — along with billing and coverage details.
  • Your provider completes the physical, including the cardiovascular exam and any form your program requires.
  • You receive the completed form to submit to your school or athletic department.

What your provider may evaluate

  • Your symptoms, how long you have had them, and what you have tried
  • Your personal and family medical history
  • Current medications, supplements, and allergies
  • Factors that may be contributing to your concern
  • Any history of heart conditions, fainting, or chest pain with activity
  • Prior injuries, medications, and allergies
  • The specific form your school or program requires

Possible treatment options

Treatment options may include prescription or nonprescription therapies when clinically appropriate. Your provider will determine whether a treatment is safe and appropriate after reviewing your health history and completing a clinical evaluation. Prescriptions are never guaranteed.

The visit documents exam findings along with any restrictions or follow-up needs. Clearance is based on the provider's independent clinical assessment and is never guaranteed.

When additional testing or follow-up may be needed

Some concerns may require laboratory testing, imaging, an in-person examination, or referral to a specialist. Your provider will advise you if your situation is not appropriate for virtual care.

If the exam finds something that needs further evaluation, your provider will discuss next steps, which may include testing, a specialist referral, or care by your primary care provider.

Travel fee and household visits

Travel fee

A travel fee applies only when a visit must be completed in person, such as a school health physical or sports physical that requires a hands-on exam. Virtual visits do not include a travel fee.

Distance (one-way from 60407)Self-pay travel feeSelf-payInsured service area*
Up to 30 miles (one-way driving)$75
Over 30–60 miles (one-way driving)$125

*Service-area eligibility does not confirm insurance coverage. The dollar amounts above apply to self-pay travel only.

  • One travel fee per household per visit — not one per patient. A household with two children pays a single travel fee.
  • Mobile visits are available at Illinois addresses only. Self-pay patients may choose either distance tier, measured by one-way driving mileage from ZIP 60407.
  • Insured patients may select the up-to-30-mile area only.
  • For insured visits within 30 miles, we seek travel reimbursement when the applicable billing rules allow. If the plan does not pay the travel charge, the clinic absorbs it; you will not be billed for unpaid travel. Normal patient responsibility for the clinical visit may still apply. Mobile availability is confirmed before scheduling and may change by plan after reimbursement review.

Frequently asked questions

Can I get a prescription through a virtual visit?

A prescription may be considered only when clinically appropriate and after your provider reviews your health history and completes an evaluation. A prescription is never guaranteed.

Can I receive care if I live outside Illinois?

Not at this time. First Priority Clinic currently provides telehealth services to eligible patients located in Illinois.

How long is a sports physical good for?

IHSA pre-participation examinations are valid for 395 days from the exam date. Your school or program may apply its own rules.

Do you perform the cardiovascular exam?

Yes. The in-person visit includes a cardiovascular exam, which is one reason the visit cannot be completed by video.

What should we bring to the visit?

The form your school or program requires, an immunization record if requested, a medication list, and any prior injury history.

Is the travel fee included in the price of the visit?

For self-pay visits, travel is separate: $75 up to 30 miles and $125 over 30–60 miles, measured by one-way driving distance from 60407. One travel fee applies per household appointment. Insured mobile visits are limited to 30 miles; the clinic absorbs unpaid travel charges. Illinois addresses only.
