Telehealth Services

First Priority Clinic, PLLC provides telehealth services to eligible patients located in Illinois. Telehealth involves the use of communication technology to deliver care when you and your provider are not in the same physical location.

What Telehealth Involves

A telehealth visit uses secure audio and/or video technology. You understand that telehealth may have limitations compared to in-person care, and that your provider may determine a condition is not appropriate for virtual care and may recommend in-person evaluation, laboratory testing, or referral.

  • •An individualized care plan
  • •Prescriptions only when clinically appropriate
  • •Referral or in-person care when needed

Patient Acknowledgments

By using telehealth services, you acknowledge that:

Care is provided to eligible patients located in Illinois; prescriptions are not guaranteed; and emergencies are not treated through telehealth.

  • •You understand telehealth is not appropriate for emergencies
  • •You understand prescriptions are not guaranteed
  • •You understand some conditions require in-person care or testing

Privacy and Technology

Appointment scheduling and clinical communication take place through our secure external Carepatron platform. You are responsible for using a secure internet connection and a private location during your visit.

Emergencies

First Priority Clinic does not provide emergency medical services. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
