Emotional Support Animal (ESA) Evaluation
An Emotional Support Animal (ESA) provides therapeutic emotional support related to a person's disability. First Priority Clinic offers clinical ESA evaluations for eligible Illinois patients who may need documentation in connection with a housing accommodation.
An ESA is different from an ADA-defined service animal. Emotional support alone does not make an animal a service animal for general public-access purposes.
Evaluation fee: $150. An evaluation does not guarantee that an ESA letter or other documentation will be issued. Any recommendation is based on the provider's independent clinical assessment and applicable requirements.

Your provider will determine whether a particular treatment is safe and appropriate after reviewing your health history and completing an evaluation. Prescriptions are never guaranteed. Certain conditions may require laboratory testing or in-person care. First Priority Clinic does not treat emergencies. Typical virtual visit: approximately 10–20 minutes; actual visit length varies.
