Coverage and network participation may vary by individual plan. Patients should verify eligibility and benefits before their appointment. A self-pay option is also available. Plan names are shown as text until logo usage is confirmed.

Standard Medical Care

Visits that generally require additional assessment or medication management, including respiratory illnesses, allergies, gout, skin concerns, stable medication refills, and non-opioid pain concerns.

Services involving additional counseling, ongoing management, or more complex clinical decision-making.

Visit fees cover the clinical visit and treatment planning. Prescription medications, pharmacy charges, laboratory testing, imaging, and other third-party services are not included unless specifically stated. Treatment and prescribing are based on clinical appropriateness.
