What the visit involves
Your provider reviews your medication history, current symptoms, side effects, and available records. Eligible patients must be located in Illinois during the visit. Some medications or situations require laboratory testing, an in-person examination, or follow-up with your usual prescriber.
Have these details ready
- The medication name, strength, instructions, and when it was last prescribed.
- Your pharmacy information and previous prescriber's contact details.
- A list of other medicines, supplements, allergies, and any recent changes in your health.
- Recent relevant laboratory results or monitoring records, if available.
Share medication records and private health information through the secure patient portal or during your visit.
