GLP-1 medications for weight management: what patients should know

What are GLP-1 medications?

GLP-1 medications act on a hormone pathway involved in appetite and blood sugar regulation. Semaglutide is a GLP-1 receptor agonist; tirzepatide acts on both GLP-1 and GIP receptors. Certain FDA-approved products, including Wegovy (semaglutide) and Zepbound (tirzepatide), are used for chronic weight management. Product names, approved uses, and instructions matter: these medications are not interchangeable.

Who may be a candidate?

For adult weight management, medication may be considered at a BMI of 30 or higher, or 27 or higher with a weight-related health condition. These thresholds do not guarantee eligibility. Your clinician also considers your medical history, medicines, treatment goals, and the specific product's labeling.

Medication can support weight loss alongside nutrition, physical activity, and other lifestyle changes. Results vary. Obesity often needs ongoing care, and weight regain can occur after medication is stopped.

Benefits come with precautions

Common side effects include nausea, vomiting, diarrhea, constipation, and abdominal discomfort. Important risks include pancreatitis, gallbladder problems, dehydration-related kidney injury, and serious allergic reactions. Low blood sugar is a particular concern when treatment is combined with insulin or certain other diabetes medicines.

Wegovy and Zepbound have boxed warnings concerning thyroid C-cell tumors and must not be used with a personal or family history of medullary thyroid carcinoma or MEN 2. This is a specific contraindication; it does not mean every thyroid condition rules out treatment. These medicines are not recommended for severe gastroparesis.

Seek prompt medical care for persistent severe abdominal pain or vomiting that prevents you from keeping fluids down. For trouble breathing or swelling of the face or throat, call 911. These are examples, not a complete list of possible reactions.

Pregnancy, contraception, and procedures

Weight-loss medication should not be used during pregnancy. Discuss pregnancy plans before treatment; Wegovy's labeling advises stopping at least two months before a planned pregnancy.

Tirzepatide can reduce the effectiveness of oral hormonal contraceptives. Its labeling recommends switching to a non-oral method or adding a barrier method for four weeks after starting and after each dose increase. Discuss a suitable plan with your clinician.

Tell your surgical or anesthesia team that you take these medicines before a procedure; do not decide on a medication hold yourself.

FDA-approved and compounded products differ

Compounded semaglutide and tirzepatide are not FDA-approved. FDA does not review them for safety, effectiveness, or quality before marketing. FDA advises using compounded drugs only when a patient's medical needs cannot be met by an approved drug.

Do not treat a compounded product as an interchangeable generic. Concentrations and administration instructions may differ, and dosing errors can cause serious harm. Ask the prescriber or pharmacist to explain the exact product and instructions. Do not buy products labeled “research use only” for personal treatment.

Have your medication and supplement list, relevant health history, prior weight-management treatments, and pregnancy or contraception plans ready. Ask about follow-up, side effects, treatment costs, and insurance coverage.
